How much can battery apps really do?

Do battery apps just monitor things and give advice? Or do they actually change what happens inside the phone? For example, can they or any software manipulate the charging rate by turning on or off a "trickle charging" mode?

I have BatteryDoctor installed. It has a setting for "Trickle Charge" that can go up to 120 minutes. The instructions seem to say this setting determines how long the phone spends charging at very low current after it has reached 100% charge at normal charging current. Is the app actually changing the current draw?
