Information gone from iPhone but is on the iCloud

Hi all,

I noticed at the weekend that all but 2 of the notes on my iPhone had disappeared, but when I looked on iCloud they are all there.
I also noticed that when people were ringing or texting it was their number coming up rather than their name. When looking through my contact list I can see that some contacts have gone but again they are all on iCloud.

Why would some notes/contacts disappear?
How do I get everything from iCloud back to my phone?
