I was having a problem with my iPhone during calls. I would mute, dial, and hang up on calls before I was 30 seconds into the call. In all my researching, calls to Apple Support, and visits to my local wireless store I found the problem. I have an iPhone protector case similar to the Otterbox and it was the source of my issue. The iPhone's proximity detector was covered by the case which caused it not to work properly. Long story short, I cut a tiny portion from the iPhone protector case and amazingly it started working properly. I am not stating this is the all powerful fix because some phones may have deeper issues, but it is worth a try. (This post is strictly an opinion, not fact.)
