Medical apps

Hello,
I am a new IPhone owner and am looking for a good (hopefully free) medical app. this app would be for an non-medical professional. I am looking for an app that will help me keep track of:
1. Current medication that I am taking
2. Medical history of procedures
3. Current physicians names and phone numbers
4. History of medications I have taken in the past.

Any suggestions would be appreciated.
