Music App starts playing randomly, no icon anywhere

My wife's iPhone 4 runnnig 6.1.
Randomly music starts playing. There is no Music Icon. It just starts playing. Only way to shut it off is to reboot. What could cause this?

My iPhone 4 also very occasionally starts to play a tune from my music library, like the previous comment I have not opened the 'Music' App icon, but when I double click the home button the App is open, I then swipe up to close and ofcourse the music stops! What could be causing this??

If the home button is hit maybe too many times, voice speak opens with a blue screen. This sometimes starts a song. Also, the music controls may be on the lock screen and is somehow being activated. Other than that, I don't know.
