There is a tweak that changes iPhone clock icon to show actual time and has a second hand. It doesn't use any of those winter board etc things. Does anyone know if using this tweak will cause any extra battery drain?

Yes, any sort of live icon or widget is going to increase battery drain since the icon is constantly changing.
