How does an App work?

Being new to these App's I was wondering how they actually work. When they've been put on the phone you obviously just press the App and away you go. Does this then mean that my phone connects to the internet?

All Apps in general. Just wondering how they work. Are they a short cut to the actual web site or are the Apps actually a small file (e.g the actual game)?
