Restored but still jailbroken?

I am not new to jailbreaking but i had a question. I wanted to restore my phone after jailbreaking my iphone 4s cuz of crashing issues. I did the restore and still see the weather icon changing and had apple id and password boxes pop up with hackers names in the username box. what else do i have to do to completly get rid of the jailbreak?>

I think, restore and setup as new phone and not from backup.

Yeah...if you restored from backup...then you didn't "truly" restore.

This is why i never restore from a backup. It's not a true restore.
