Connect your iPhone to the computer, open iTunes and wait for it to detect it. Then select your device, and select "Summary". There, you'll find "Backup Now", and right next to it "Restore Backup" - that's what you choose. Select the backup with your data included, and click "Restore".

I noticed that the only iTunes backup I am able to choose is from the 21st.
It was the 21st when I lost what I'm trying to get back.

When you select your iPhone's name here:

You'll get other backups as well.
