Randomly lost contacts phone #

CAn anyone tell me why some of my phone #'s randomly disappear & how to prevent it? They usually save the second time I add them to my contacts. .

When you put in a Contact, do you then hit the SAVE button?

I hit the done button @ the top right

When someone calls that I don't have in my contacts I create new contact & hit done. Days later I try to contact them & they are no longer listed in contacts. So I then go back to my received calls & try to find it to add it again.
